ANTI-AGING SERIES – PART 4
By Mark Van Wormer, MD, ABAAM

Balanced Hormone Replacement Therapy

The essence of anti-aging Medicine is:

1. Reducing risk factors and preventing degenerative diseases with periodic physical exams and healthcare screenings and by limiting high-risk behaviors.

Screenings include: colonoscopy, bone density studies, pap smears, mammograms, prostate exams, lab work, etc.

High risk behaviors include eliminating smoking, using seat belts, etc.

2. Eating healthy, low glycemic foods, using balanced supplements (nutraceuticals) and drinking adequate water.

3. Getting enough exercise and sleep.

4. Balanced Hormone Replacement Therapy.

Why Hormone Replacement therapy?
Hormones are the driving chemicals that tell our bodies how to behave. As we age, hormone levels decrease. This leads to the loss of healthy cellular function.

Do we really have to age at all?
Of course, but anti-aging therapy can slow the aging process. You can experience a marked improvement in many aspects of your physical/mental abilities by following an anti-aging program that includes balanced hormone replacement therapy. Quality of life improves as physical stamina, mental stamina and ability to interact in this complex world gets easier.

Diminishing Hormones

Melatonin
As we age, our hormone production diminishes. The first hormone to diminish significantly as we age is Melatonin.

Melatonin is a powerful antioxidant; human chromosomes have melatonin receptors which prolong their lives. Melatonin prevents destruction of chromosomes by oxidation, although we are not sure how.

Melatonin deficiency has been hypothesized as one possible cause of breast cancer.

Melatonin is a hormone which aids in falling to sleep and inducing deep restful sleep, which is when the body can repair itself.

At about the age of ten, our pineal glands (a deep seated gland inside the brain) start to deteriorate. As a result, Melatonin production diminishes.. Babies and young animals sleep two thirds of the time; as adults we seem to require less and less sleep: we even have great difficulty falling to sleep. Even when we do sleep, we still don't get the correct type of sleep.

Human Growth Hormone (HGH)
The next hormone to go is Human Growth Hormone, which starts to drop off in adolescence. Lack of this hormone is called "somatopause."

Human growth hormone is what causes growth in children, but it also has numerous effects in adults. HGH stimulates the liver to produce powerful immune stimulants, decreases body fat, increases lean muscle mass, increases aerobic endurance and works with sex steroid hormones to augment their powerful effects.

Numerous studies have demonstrated the powerful results of daily injections of small physiologic doses of Human Growth Hormone (about one unit daily subcutaneously).

Testosterone As males age, they go through "Andropause," where testosterone levels decline, usually in the mid thirties. The result is loss of strength, increased body fat, decreased muscle mass, decreased libido, increase in cholesterol and risk for heart attack, lack of drive and decision making capacity. Male "couch potatoes" are usually low in testosterone.

Replacing testosterone to levels found in normal 26 year olds results in building lean muscle, loss of body fat and "love handles," increased libido, increased ego strength and command of the environment. When testosterone replacement is combined with Human Growth Hormone Replacement, the effect is synergistic (greater than either hormone alone).

Andropausal men lose on average 15-18% body fat and replace it with lean muscle mass while undergoing combined therapy.

Estrogen & Progesterone
As females age, 'menopause" occurs at about age 45-50. Ovulation ceases and estrogen, and progesterone levels plummet. Muscle mass diminishes, just as in males, libido decreases, female physical characteristics diminish, osteoporosis begins, memory problems, hot flashes, mood swings and all the well known attendant problems of menopause occur.

Replacing hormones to levels found in 26 year olds reverses most of these symptoms. Muscle mass increases, body fat does not decrease unless Human Growth Hormone is added, secondary sexual characteristics return, libido returns, exercise tolerance improves, improved thinking and well being occur.

Other Horomones What other hormones can be replaced to effect even greater change?

DHEA, a powerful adrenal hormone and sex steroid hormone precursor is also a powerful antioxidant and stimulant of improving lean muscle mass and decreased body fat. Again, replacement is never supraphysiologic.

Pregnenalone, another adrenal steroid precursor of some sex steroids, has been shown to be a memory-enhancing agent.

How do we know how much to replace?
Frequent monitoring of hormone levels through blood work (about every 3 months) allows physicians to customize replacement to be optimal for a given patient.

In summary, replacing Melatonin, DHEA, Pregnenalone, Testosterone, Estrogen, Progesterone, and Human Growth Hormone in concert with a low glycemic diet, exercise, nutraceuticals, adequate hydration and sleep can result in reversal of the "normal' aging process and restore incredible health to almost anyone.

Of course, the prudent anti-aging physician monitors many other variables than just hormone levels. In fact, age management is complex endocrinology. The improvement in strength, muscle mass, reduction in body fat, improved immune status, improved healing capacity and resistance to illness, increased memory and functional capacity are all quite impressive in a well controlled carefully balanced Anti-Aging Program.

Correcting Pigmentation Problems

Whether you have taken good care of your skin or not, by the time you are in your forties, you may notice the appearance of brown, freckle-like spots on the back of your hands, on your wrists, your forehead, upper cheeks or upper lip.

The slowing of the cell renewal rate that is basic to aging causes these pigmented spots. The spots most commonly turn up when skin cells divide. So, gradually they pick up nearby pigment producing cells. Sun exposure, lack of sunscreen, stress, birth control pills, pregnancy and certain medications contribute to their appearance, as well.

The following skin lighteners used in conjunction with sunscreen can help fade unsightly pigmentation:

Hydroquinone, the most powerful lightener, decreases melanin production quite well. When pigmentation is superficially located, depigmentation can be expected in two to four week's time. Deeper pigmentation may require up to three months and the effect may be incomplete.

Hydroquinone in higher concentrations is only available under prescription and is most effective if used with Retin A. It is not approved for children under 12 years of age and should be avoided during pregnancy.

Be advised that prolonged use over six months may lead to "rebound pigmentation" - a return of the pigmentation problem to a greater degree. Another drawback is that some people are allergic to hydroquinone, so before beginning use, perform a "patch test" by applying a small amount (the size of a pea) behind the ear to see if you have a reaction to the product.

A safe, and effective alternative to hydoquinone is Kojic Acid, which is a mild antibiotic derived from aspergillus and can effectively inhibit melanin formation. Both products that are part of our special this month contain Kojic Acid. We find these products very helpful for reducing hyperpigmentation.

Remember, a lightener used without a sunscreen will have little or no effect. A sunscreen with ultra violet ray - blocking agents such as zinc oxide and titanium dioxide should be applied every day over the lightening agent to attain the most optimal results.

The GreatSkin line has three effective sunblocks with titanium dioxide that are easy to use. Check them out on our Solar Care page.

Obagi Sunblock contains both Titanium Dioxide and Zinc Oxide. You'll fine it here.
