BEAUTIFUL LIPS

Our lips are probably one of the things that we most take for granted. Like our skin, our lips need to be exfoliated, hydrated, and protected, to ensure having the pretty lips we so desire.

Exfoliation
Start by buying a soft bristled toothbrush to keep on hand just for your lip exfoliation. Wet it, and apply a small amount of an exfoliant like Bioelements Measured Micrograins and gently brush the lips. With a moist washcloth, remove and rinse all particles off. This can be done 3 times weekly while exfoliating the rest of your facial area.

Hydration
Whether you wear lipstick or not, you should wear an emollient lip balm like Bioelement’s Instant Lip emollient or one of Joey’s "Delipcious" Lip Balms SPF15.

Solar Protection
Keeping the lips protected from the sun is imperative. Lips are much prettier when not covered with brown spots from sun damage or fever blisters. Try Biomedic’s Lip Shield SPF15 to help guard against such damage.

Anti Oxidant Protection
After 40, keeping the lips young looking is a challenge. At this time, the lips start to change and aren’t as full and voluptuous as they used to be. Extreme Weather Protection

SEGMENTAL FACIAL ENHANCEMENT, PART I PERI-ORAL AND LIP ENHANCEMENT

The aging face may begin showing signs around the mouth and lips. Signs of aging include loss of lip volume, those fine vertical lines about the lips that slowly weave their way into the vermilion border of the upper and lower lip as "bleed lines," where lipstick tends to "bleed " into the line and accentuate it. The oral commisures are the grooves on either side of the mouth, which slowly turn down with time as gravity wins out over the loss of collagen in this area, giving one a "permanent frown." These lines are sometimes referred to as "drool lines," sadly enough. The textural changes of the skin including dyschromia (pigmentation around the mouth area), coarseness of the skin, enlarged pore size, actinic damage from the sun all combine with dynamic lines and solar induced photoaging to result in the "Aging Mouth." Not a pretty picture, since most people stare at other’s mouths when they speak in order to hear AND see the spoken word for reinforcement of the dialogue.

What are the current treatments for the Aging Mouth?

First, volume loss of the lips generally results in the post-menopausal period as a result of estrogen lack and generalized hormonal demise. As an anti-aging physician, I will always measure blood estradiol, estriol, estrone, progesterone and testosterone levels and supplement with transdermal bioidentical human hormones.

Hormone replacement therapy has received some very negative attention lately. The essence of the argument is that the combined use of conjugated (the biochemical liver detoxification process resulting from oral intake) HORSE estrogen and excessive doses of progesterone taken orally and TOGETHER result in an increased risk of heart disease events (7 more per 10,000), breast cancer (8 more per 10,000), stroke (8 more per 10,000) and pulmonary emboli (8 more per 10,000), but 6 fewer cases per 10,000 or colorectal cancer and 5 fewer hip fractures. These studies were performed only on patients taking the popular drug PremPro® and not on those taking bioidentical transcutaneous therapy. Many studies have looked at the risks of bioidentical therapy and not found any of the above risks to occur, but substantial improvement in the risks for heart disease, stroke, improvement in vasomotor symptoms and osteoporosis.

INext, I will augment the lips to subtly give them more volume, either through collagen injection or Alloderm® strip implantation. Collagen augmentation will last about two months and should be repeated frequently to form a "collagen base," to continue to build on, while Alloderm® strip augmentation is performed no more than yearly.

Regarding the fine vertical lines of the upper lip. I am asked endlessly about what to do here. First, the approach must be combined. I use small doses of Botox® placed around the mouth to gently paralyze some of the muscles of facial expression which result in the vertical lines. With carefully placed Botox® the oral commisures ("drool lines") on either corner of the mouth can be turned up by paralyzing the muscles which pull them down. Botox® can be used in the chin to stop dynamic lines that look like cobblestones when making certain facial expressions. Second, we prescribe a course of microdermabrasion therapy to simulate collagen and resurface epidermis. We also have many chemical peels, which can result in continuous epidermal exfoliation. Third, we may resort to actual collagen injection into the lines themselves, to "plump them up." Fourth, we always recommend non-ablative laser therapy (the Aramis® 1540 laser) continuously used on a monthly basis, to stimulate dermal collagen production over a period of time. In fairly marked cases of peri-oral aging, i.e. very deep lines, I may suggest peri-oral CO2 resurfacing, a very effective way to "start over," with only about 5-7 days of down time.

The home care products for peri-oral, lip and face enhancement are many. The active ingredients should contain DMAE, alpha Lipoic Acid, Glycolic Acid, Tretinoin (Retin A®), hydroquinone and kojic acid, and topical Vitamin C.

There are now many fine office based, no down time treatments to improved skin texture, lip size and texture, and line presence and development around the mouth. Treatment of the Aging face continues to be less invasive and less difficult to perform as advancements such as Botox®, non-ablative laser (Aramis®), microdermabrasion, Collagen and Cymetra® (human collagen), and quality product development become more widespread.
